The memorial is a bronze plaque, dedicated in honour of national servicemen from the Northern Rivers of NSW who died during the National Service period from 1951 to 1972. It features the emblems of the Australian Army, Royal Australian Navy, and the Royal Australian Air Force. The plaque is mounted on a concrete plinth within Lismore's Commemorative Park, along with a number of other memorials. A flagpole is also positioned behind the memorial. The park is the venue for various commemorative services held throughout the year.
