The Coolatai Anglican Church First World War Honour Roll is a carved timber board, made of maple. It has an overall rectangular shape, with a curved pediment at the top featuring a carved Rising Sun emblem with a crown underneath. It also has small supporting shelves at the top and bottom and carved columns on each side. The memorial bears the names of Lance Sergeant George Albert Cawkwell and Lance Corporal Jesse Judge, who died in the war. It was unveiled at the Coolatai church in April 1922 and dedicated by the Reverend R.H. MacLean (Warialda Standard and Northern Districts' Advertiser, 24 April 1922). The memorial is located in the Coolatai Anglican Church and can be viewed when it is open to the public for services.
